Nursing Home
Our Nursing Home provides compassionate, 24-hour care for seniors who require ongoing support and are unable to be cared for at home due to varying care needs and circumstances. Our multidisciplinary Care Team comprises nurses, medical doctors, allied health professionals and support staff who work together to provide holistic care, including nursing, medical, rehabilitative and pastoral support.
We are committed to delivering person-centred care in a dementia-friendly environment, where each resident is respected and supported as an individual. Residents are encouraged to participate in meaningful daily activities, reminiscence therapy and social programmes that promote physical, emotional and social well-being, while fostering a sense of dignity, purpose and belonging.

Senior Care Centre (SCC)
Our SCC offers a comprehensive range of centre-based eldercare services to support seniors with varying care needs. We aim to enable seniors to age-in-place with dignity and independence, while providing caregivers with greater peace of mind and respite.
The SCC provides a convenient, one-stop service offering:
- Maintenance Day Care (MDC): Supervised day care that supports seniors in maintaining their physical, cognitive and social well-being, while providing caregivers with support and respite during the day.
- Dementia Day Care (DDC): Specialised care and support for seniors living with dementia through structured cognitive, therapeutic and social activities. These programmes aim to maintain functional abilities, manage behavioural symptoms and enhance overall quality of life.
- Centre-Based Nursing Services (CBNS): Nursing care for seniors with ongoing clinical needs, including medication management, health monitoring and wound care, to support their continued health and well-being.

Gym Programme for Seniors
Our gym programme offers seniors from all walks of life a safe, enjoyable way to stay active and maintain strength, mobility and independence. Using specialised gym equipment, the programme provides gentle, guided exercise tailored to individual abilities and needs. With regular sessions, participants can work on balance, coordination and everyday functional movement in a supportive and encouraging environment.

Q1. What is the process for admission to a nursing home?
Ans: We serve clients/residents who are mostly referred by the Agency for Integrated Care (AIC). AIC and the Medical Social Worker (MSW) of the restructured hospital/polyclinic where the elderly is receiving treatment will jointly assess the elderly for admission to a nursing home. The referral process is completed only when the elderly is accepted for admission to a nursing home.

Q3. Does LKH admit any residents who are not referred by AIC?
Ans: LKH has a limited number of non-subsidised beds for those who are not referred by AIC. Private residents are accepted strictly on a case-by-case basis.
